The undersigned hereby waives any and all rights and claims for damages against the Bermuda Underwater Exploration Institute, BUEI Camp, BUEI’s After School Programme, staff and volunteers. The applicant must be of sound physical health and capable of participating in BUEI’s programmes. In the event of injury or sickness, the Coordinator and staff have my permission to arrange for medical care and will contact me as soon as possible. The BUEI reserves the right to terminate the registration of any participant when it is deemed in the best interest of the child or the Camp, or After School Programme. *

The undersigned gives permission for their child to participate in photography/video production that is used internally and externally for promotional purposes and for BUEI to have exclusive reproduction rights to these images. *
